How much filler should be used?

One of the most frequent challenges for less experienced injectors is deciding how much filler is the right amount. Many newcomers opt to ‘play it safe’ by under-treating, fearing that too much filler will produce an unnatural or ‘overdone’ look. However, under-treating often leads to subtle or disappointing results that don’t fully meet patient expectations.

Based on extensive clinical experience, Natalie recommends starting with a minimum of 2ml for mid-face volumisation intended for restoration and rejuvenation. This initial volume allows for a meaningful lift and contouring. We then wait about four weeks for the filler to settle and integrate fully into the tissues. After this period, the area is reassess to decide if additional filler is needed, allowing for fine-tuning in a staged manner.

This conservative yet structured approach helps avoid the need for dissolving product and reduces risks associated with overfilling while optimising the final outcome.

Why is precise placement critical?

The location and depth at which filler is injected directly impacts both safety and aesthetic results. Cheek volumisation primarily involves placement into the deep supraperiosteal plane, the layer just above the bone. Injecting filler here restores the foundational support that diminishes with age.

In some cases, especially for rejuvenation, filler may be placed into more superficial layers where fat pad atrophy has occurred. However, superficial injections require even greater precision to avoid lumps, irregularities or unnatural contours.

Incorrect placement or injecting too superficially can create visible bumps, asymmetry or the so-called ‘pillow face’ effect, which is often caused by excess filler at the apex of the cheek. This is why a thorough facial assessment and detailed anatomical knowledge are essential. Our clinicians carefully map each patient’s facial structure before proceeding.

Assessing the face as a whole

Volume loss or hollowing is rarely isolated to the cheeks. Areas such as the temples and in front of the ears (medical term: preauricular zones) contribute significantly to mid-face aesthetics. Treating the cheeks alone without addressing these adjacent regions can inadvertently lead to an unbalanced or aged appearance.
